Set up your Lead Capture Form
Add a quote request form to your website or share it as a direct link, and turn every submission into a new lead in your dashboard.

The Lead Capture Form is a quote request form your customers can reach any time of day. You can embed it on your website or share it as a direct link, and every submission comes into FreshFSM as a new lead. This guide covers turning the form on, choosing what appears on it, and getting it in front of customers.
Setting up your form
All devicesYou set everything up from the web dashboard. It takes a few minutes, and you can change any of it later without touching your website again.

Turn the form on
Open Settings and go to Lead Capture Form, then switch on Enable Form. The form stays private and cannot be submitted until you turn it on.

Choose which services to show (optional)
Under Services, tick the services you want customers to be able to request. This step is optional. If you do not select any, requests still come in as a general quote request. If the list is empty, add services first under Products & Services.

Customize the look
Set the heading, description, and accent color so the form matches your brand. The live preview updates as you type. When you are happy with it, click Save to publish your changes.

Add it to your website
In the Embed & Share section, copy the Website embed snippet and paste it into your site where you want the form to appear. The form resizes itself to fit, so you do not need to set a height or worry about it being cut off.

Or share a direct link
Do not have a website, or prefer not to edit its code? Copy the Direct link instead and share it in an email, a social bio, or behind a button. Use Open the form in a new tab to try it yourself first.

How to test your form
- Open Settings, go to Lead Capture Form, and confirm Enable Form is on.
- Click Open the form in a new tab, or open your direct link.
- Fill out the form with your own details and submit it.
- Check that a new lead appears under Clients and that you received the notification email.

Each submission creates a new lead in your dashboard along with a quote request, ready for you to follow up. If the email or phone number already belongs to one of your clients, the request is attached to that client so you do not end up with duplicates. The customer also receives a confirmation email automatically.
